Rosa Yesenia Hernandez, RN

Owner/Administrator

Rosa Hernandez is a registered nurse with over 30 years of medical experience. Rosa was raised by her grandparents in her native country of El Salvador, as her mother migrated to the United States after her father passed away when she was three years old. At age 10, Rosa’s grandmother became ill and passed away right before her eyes while she was home alone. She recalls holding her and felt as though their hearts connected into one. This was the turning point where a future nurse was called upon. Feeling helpless to the situation, she made a promise that one day, she would help many others in her honor.

At age 12, her mother returned and legalized her migration to the United States with Rosa’s three other siblings. Hernandez stepped into a whole new world, not knowing a word of English. She began school upon getting to the United States, and with her determination to learn the language, she was able to speak English within a year. Rosa attended Immokalee High School, where she became a single parent of two, but this was not an obstacle to keep her from climbing the ladder, she graduated in 1992. She then enrolled herself into James Lorenzo Institute in 1993, where she obtained her certified nurse assistant (CNA) certificate and began to work at NCH. She attended night school to receive her licensed practical nurse (LPN) degree and graduated in 1995. While working as an LPN, she gained experience in hemodialysis, oncology, hospice care (end-of-life) and home health care. Rosa then met her husband, Giraldo, who encouraged her to go back to college. She graduated from Florida Southwestern State College in 2004 with a registered nurse degree (RN). Soon after obtaining the license, she applied at NCH and was offered to work in the progressive care unit (PCU) due to her bilingual ability and later was promoted to the intensive care unit (ICU) after completing the internship. She was awarded the Rising Star Award in 2007.

Hernandez then worked in the Collier County Public School system as an NCH nurse, where she was awarded the Daisy Award in 2012. The Daisy Award is an international program that rewards and celebrates the extraordinary clinical skill and compassionate care given by nurses every day.

Rosa, or Yesenia (as she likes to be called) and her husband decided that they were ready to grow their family and had two more children. Currently, she is a proud grandmother of three.

After serving the Collier County community where she has lived since 1984, Hernandez decided she would bring her childhood promise to life, as she never stopped striving to give her best care to others. The knowledge and experience she has gained so far is the paramount of Rosa‘s healthcare passion. Rosa’s authenticity, emotional support, medical care and love for humankind is what sets the rhythm of Heart2Heart Concierge Home Care.
